Avance Care Mallard Creek takes a comprehensive patient-centered approach to weight management that prioritizes your total health and wellness. Our weight management program is designed to support individuals aged 18 and over, including seniors, in achieving their weight management goals through a multidisciplinary approach with a team of dieticians, therapists/coaches, and clinicians.
We recognize that obesity isn’t just about weight—it’s about managing a chronic disease that impacts your overall health and quality of life. Our goal is to empower you with the knowledge and tools necessary to effectively manage your weight and prevent or mitigate obesity-related diseases such as hypertension, diabetes, high cholesterol, sleep apnea, and more. Whether you’re dealing with weight regain after bariatric surgery, weight gain from medications, navigating perimenopause / menopause, or facing challenges related to mental health, our program is here to support you every step of the way.
The Avance Care Mallard Creek Weight Management program is designed to help any patient with a BMI of 30 or greater, or those with a BMI of 27 or greater coupled with chronic diseases like diabetes and hypertension. We understand that BMI is not the only indicator of health. Our clinicians will perform a comprehensive assessment that includes reviewing your medical history, previous labs and diagnostics, clinical exam, body composition testing, and determine additional diagnostics that may be needed. Our program is tailored to meet your specific needs.
Multidisciplinary Obesity Care Team Members
An effective weight management program requires a multidisciplinary approach tailored to everyone’s unique needs. A team of dedicated professionals work collaboratively to provide comprehensive care and support for individuals struggling with obesity.
Your primary care provider is often the first point of contact in managing obesity. They play a crucial role in assessing your overall health, identifying risk factors, and coordinating your care with other specialists as needed.
Your primary care provider may not always be a physician. Physician Assistants and Nurse Practitioners work closely with physicians to deliver high-quality care and support to patients. They perform physical exams, order tests, prescribe medications, and provide education and counseling. PAs and NPs play a vital role in ensuring continuity of care and are available to address any questions or concerns you may have throughout your journey.
